NEED HELP ASAP - Amendable PDF

Hi Guys, it's been a while.

I need some help. A client of mine has asked me to design a amendable PDF form/survey which will be e-mailed out to their clients, filled in and e-mailed back to them.

I thought it would be fairly straightforward, I created the survey in Illy CS (questions on the left, blank spaces for the info down the right), saved it, used the 'touch up text tool' to add my name, title etc in Acrobat but once I came to fill in my address that's where all the problems began. As soon as I hit the return key to enter the second line of my address the text started jumping all over the place :-(

So is it possible to create a true amendable PDF survey without it going all weird?

I think that is what Adobe Designer is for.

Just looked into it and that's exactly what Adobe Designer is for.

It doesn't matter guys I've sorted it out, I converted the questions to outlines and left the blank spaces as editable text, seems to have done the trick for now... I hate doing things half arsed

Thanks Mynock I thought there would be some sort of software out there for the job, it's just a shame I haven't got access to it.
