folding resume CD holder

i'm looking for an eps, quark or any vector work that will produce a "+" floding cd holder.
*CD in center pouch, and 4 sides that fold in covering the entire CD, with each fold having a different part to my resume*

i'm working on a resume since graduation is close and have a domain of *myname*.name registered, so i want to get things rolling and have not just a job, but a good job lined out before i wear a cap and gown.

i have full access to a plotter, edge and other tools, so that isnt a problem at all.
