Nice job, just a few little details:

-Try putting a bit of a bulge in the eyelid where the needle goes underneath it.
-There are a few extra pixels at the tip of the needle, it might be just a freckle, but the placement of it detracts from the piece, so get rid of it.

At some forums that image wouldn't lift a single eyebrow, so I suspect that I'm really bad at spotting irony, or that the adults of GDF hadn't had the "pleasure" of seeing all the things the Internet has to offer.

I like it.. I think the puncture could be a little more convincing..

Also the needle could be perhaps blurred (gaussian maybe?) to help it blend with the background photo so it doesn't appear so sharp.

Really interesting idea though.

As far as being shocked ..... no ... nothing shocks me anymore. I have been around the net and back.... but as Urst said.... some things just freak me out ... anything to do with the eyes and spiders.... Now had that needle been through any other body part I probably wouldn't have had any reaction at all.

I used to hang out at "GFX forums" where people just whip up signature after signature, and they found these kinds of things kind of funny, so they posted similiar stuff, both manipulated and real. A lot of "ownage compilation"s also contributed to my immunity to these things :P

I did a sculpture in high school that was very similar. I titled it "the eye of the needle." It was pretty cool. Wish I had a photo of it. It got washed away in Katrina.
