Some of my pics have been used in magazines, and in ads. I've also made one ad myself for a summer magazine for a company. I am also working on a box now for sports.
Now I wonder if my name or website should be on all of this or not? As the creator of the ad, or as the photographer. Maybe a silly question, but I would really like to know what is common in this. As till now my name hasn't been on it, but I wonder if that might be wrong...

Depends on your usage contract on the images and how they are used. For ad work and general usage there is usually no credit given (there are always exceptions, ie the museum design industry always uses photo credits). Pick up a few magazines and flip through them. How many photo credits/studio credits do you see in the ads? It's not your ad, it's theirs.

i've never included my info on artwork i've done... unless it's an advert for myself obviously. I've had a couple of photos used in magazines too and whilst being credited to me in the text, it's not appeared on the image.
and the only things i've seen the info of others feature on, is party invitations, event programmes or cheap restaurant menus.
Personally i think 'designed by...' text on something instantly lowers the value of the artwork and make's it look a little cheap.

As PrintDriver mentioned, it all depends on your usage agreement with the client. There have been times in my career where a design or illustration credit has been part of my compensation package with a startup client - and it has added a great deal of value to the project for me. I have no problem asking for a such credit on ads, programs, invitations, etc, for nonprofit events - especially in pro bono situations. I've also known a number of web design/development firms who have been given such credits/links on sites by very satisfied clients. I don't think it cheapens or devalues one work at all - it's just a viable alternative form of marketing and promotion.

maybe. having thought about it, i think it's probably because i've ONLY seen 'designed by' straplines on the cheaper things like school fayre programmes etc, i just associate it that way. I guess it can always be done subtley, tastefully and still look quite professional.
