I need to write a performance review for one of my junior designers. Does anyone have a template or a review that you have used in the past. I would really appreciate it as I am having a hard time finding one that relates directly to graphic designers.

actually is really simple. lol
(rate yourself/employee on a scale of 1-5, 1= horrid — 5= Amazing) Give a brief paragraph about each as well.
Communications
Customer Service
Dependability
Initiative
Integrity/Ethics
Interpersonal Skills
Job Knowledge
Quality of Work
Quantity of Work
What have been your major accomplishments this year?
What training and development would you like to receive in 2007 that you feel would improve your performance or enhance the opportunity for career development?
List the 2007 goals and objectives you will like to accomplish.
Thats about it... Super easy and to the point. Hope it helps.

Another thing is breaking down the actual JOB DESCRIPTION. if you don't have one, good time to create one.

personally I like the idea of self reviews, this gives one the opportunity to rethink where they have come from, what they have done, and where they are going
and, gives the actual tudor or mentor the opportunity to look inside how this person feels about themself and their progress

Another way is for you both to do one and then you meet and disscuss their performance. Whereever you and the person come up with um.. rather large discrepancies... its easy to pin point if theres a problem or issue that needs to be addressed.
Just another suggestion for consideration.

another good thing about them is it puts the review-ee on a less defensive and more prepared reaction.

You are all scaring me. I have always hated (down right loathed) writing essay-type things, especially about myself. Literally, it makes me sick.
Don't get me wrong, I'm capable of good writing, it's just getting started is overwelmingly intimidating. Also, I tend to be hype-critical of myself.
At FMI we had to type a list of the projects we'd worked on throughout the year. Then the Art Director would give the review. I always did well, and got raises. At DecoArt, we were lucky if we got a review at all, and while I was there we only got something like a 2.5% raise.
