I need to write a customer satisfaction survey to send out to some of our customers... the goal is toevaluate the way our service technicians go about the job and hopefully streamline efficiency of service calls.

I realize that the survey has to be relatively short, simple to understand and respond to, and that it should be sent with a self-addressed, postage paid envelope to encourage people to return it. We will also offer a small incentive for those who DO actually complete and return the survey.

Do you guys have anyotherideas of how to make this effective and encourage response?If anybody has ever doneanything related before I'd love to hear from you!

i would say a post card that they can fill out and drop in the mail. Short simple and they dont even have to stuff it!! :o

that's a good idea, but we'll probably print them ourselves... and if you give me a ruler and a razor blade to cut the blasted things apart I'll definitely lose some fingers!

I saw this in a design annual a few years back - simple and to the point

You know, there's like a butt-load of gangs on this forum. This one gang kept wanting me to join because I'm pretty good with a bo staff.

Yeah ... a postcard is a good idea. A publishing studio I worked for did that, featuring limited edition postcard of their artists's work, and posted it out in really fancy envelopes - image can be everything, when you want to attract big bucks.
