Forms in Illustrator, or something other than Acrobat?

OK, I've got the new Acrobat 7, and in this edition, Adobe has taken the form tools from Acrobat and instead allows users to use the more robust program Adobe Designer (or something like that). The program looks fantastic, but I don't have time to learn how to make it work (I'm on a deadline, as always). What I'm looking to do is create a form like I used to be able to in acrobat, with drawing out the boxes around a template, and saving it as a 1.5 pdf. The problem with Designer is that it only saves it as a 1.6pdf, and when I draw a box, it won't let me draw it the size I need it, it adds a Caption to it that I have to delete, and basically doesn't let me format it the way I want. Now, I'm sure that Designer is able to do all the things Acrobat forms can and more, but right now I don't have time to learn it. So what I want to know is if there is a way to design a form (with filliable form fields using variables) in Illustrator, or Indesign? What I'm doing is for an online catalog. I have a template business card that people can go and type their information, then see a proof of the pdf with their information on the card. But I need a "form" 1.5 pdf in order for it to work. Any suggestions?
