Work Order/ Docket creation

I am looking for a way to create custom work orders or docket forms that can be accessed online and generate a database of submitted forms in an archive. I don’t want a project management software just a simple way to create work orders and save them. Having auto-population would be great too for repetitive entries. The work orders would be anything from small format printing to bus wraps to online marketing to web site design and development. Any recommendations? Free to cheap is preferred.
